Music 6000 Band Rentals-Rental Application

• Filling out this form online will speed the process for you in the store.  
• After the form is received you will get a confirmation EMAIL
• All rentals are first come first serve.  Please feel free to call the store and see what we have in stock.
• You will need to sign a contract and bring a valid Washington State ID and Credit or Debit Card. All payments for rentals MUST be made with the card you bring in. We cannot accept cash or checks for rental payments. The first months rent is due in full at time of pickup.
• The form below is encrypted and sent securely.

We are currently out of or have very low stock of Alto Saxophones, Tenor Saxophones, Clarinets, Flutes, Trumpets, Trombones, 4/4 size Cellos, and 4/4 size violins.  Please call the store to check on availability before you make the trip in.

Completing this form does to guarantee an acceptance of your application. All rentals are at the sole discretion of MUSIC 6000 Inc. management.
